Coordinated Accommodation Requests

 

To make a request for a coordinated accommodation, you must have a current Accommodation Plan with the Office of Disability Resources which reflects the approved accommodation.

 

If you are not currently approved for an accommodation related to the below forms, complete an Additional Accommodation Request Form. ODR will then review documentation to determine eligibility.
